Purpose
This tool generates a worksheet for monopolar review (single-contact screening) during DBS programming. It is intended as a documentation aid to somewhat ease what can be an onerous process.
Build the sheet
- Set the number of contacts you want to screen.
- Set the amplitude ladder — start, increment and maximum. Turn the ladder off to get blank rows instead.
- Set the number of assessment columns.
- The worksheet below the controls updates as you go.
Fill or handwrite
Every cell is a text field you can type into. You can fill it in on screen and print it, or print it empty and write on it, or a combination of both.
Print and export as a blank sheet leaves your entries on screen but keeps them off the printed and exported copies. Column headings and contact names are treated as part of the template, so they are kept either way — a configured sheet can be reused for the next patient.
Clear entries does the same thing on screen, also keeping headings and contact names.
Output
PDF is produced through your browser's own print dialogue, so it prints at true A4.
The Word export is a .docx written in the browser.
Privacy
Nothing you type is transmitted, stored or logged. Only configuration settings are saved locally, should you tick that option
